WE DO NOT HAVE ANY AGENT FOR MISCELLANEOUS SERVICES. APPLICANTS CAN SUBMIT THEIR MISCELLANEOUS APPLICATIONS IN PERSON AT THE COUNTER AT THE EMBASSY OF INDIA, CONSULAR WING, 2536 MASSACHUSETTS AVE NW, WASHINGTON, DC 20008 BETWEEN 10AM AND 12.30PM ON ALL WORKING DAYS (EXCEPT EMBASSY HOLIDAYS) OR SEND APPLICATIONS WITH ALL DOCUMENTS BY POSTAL MAIL TO THE EMBASSY OF INDIA, CONSULAR WING, 2536 MASSACHUSETTS AVE NW, WASHINGTON, DC 20008.

There is no parking space available and the nearest Metro Station is at Dupont Circle, which is approximately 1 mile away from the Consular Wing. 

Please carry originals of all the supporting documents which you have enclosed with the application.The originals have to be shown to the Consular Officer. If applying by mail, Photocopy of all the supporting documents should be notarized beforehand.

Please apply only to Jurisdiction of Embassy of India, Washington D.C. (Bermuda, Delaware, District of Columbia, Kentucky, Maryland, North Carolina, Virginia and West Virginia).

BIRTH CERTIFICATE

Birth certificate is issued on the basis of entries in your Indian passport. Therefore Birth certificate can only be issued if an Indian passport in original is presented at the counter or through mail.

Non availability of birth certificate can be issued to those Indian citizens who were born in India before 2nd June 1969 and present their Indian passport in original at the counter or in mail.

Documents required when applying in person:  Please have the following documents ready before you come to the counter

1 . Miscellaneous Services Form - Download the Miscellaneous Application form and fill it up carefully. Each applicant is required to fill up separate Miscellaneous Application form and affix his/her photograph on the same.

2. ORIGINAL Passport and Photocopy of the first five pages and last two pages of the current passport.

3. ORIGINAL and Photocopy of any one of the following as proof of your US Visa Status

(Please carry originals at the counter):

o Page containing visa on passport (H1B, H4 etc) and the copy of I-94

o Clear Photocopy of Green Card

o Employment Authorization Document (Work Permit)

o I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being made to regularize status.) 

4. ORIGINAL and Photocopy of any one of the following as proof of your current US residence address.

o U.S Driving license.

o Gas, Electricity, Water or landline telephone bill displaying applicant’s address

o House Lease Agreement

o State Identification Card

o Note: Bank/credit card/mobile phone statements are not accepted as residence proof. 

• If the applicant is holding a B1/B2 visa and your children have applied for your Green card, the applicant should provide some additional documents. Your son/daughter’s current Indian passport copy, his/her driver’s license copy, if son/daughter is a US citizen then, his/her old Indian passport first and last page copies, documents from USCIS requesting Birth certificate, receipt notice from USCIS which shows that Green card has been filed and lastly a letter from him/her to the Consular officer requesting this service for his/her parents.

• Fee: A non- refundable fee of $25.00 per copy.  Each applicant for Miscellaneous Services has to give a separate money order/cashier’s check / cash of US $2 (two) drawn in favor of Embassy of India- Washington DC for Indian Community Welfare Fee (ICWF) w.e.f. September 1, 2017 .
Personal checks, credit cards or other banking instruments are not accepted.

• Current passport in original and photocopy of the first five pages and last two pages of the current passport. If or any other country passport holder, please submit your old Indian passport in original also.

• Notarized photocopy of any one of the following as proof of your US Visa Status:
o Page containing visa on passport (H1B, H4 etc) and the copy of I-94
o Clear Photocopy of Green Card
o Employment Authorization Document (Work Permit)

• Receiving back your documents: You can send either a trackable prepaid return mailing envelope from UPS or FEDEX etc. or enclose a separate money order/cashier’s check for $ 20 (twenty) payable to Embassy of India-Washington DC.
