Applicants are advised to submit their passport application in person at Embassy of India, Consular Wing, 2536 Massachusetts Ave, NW, Washington, DC 20008 BETWEEN 10AM AND 12.30PM or send by postal mail to Embassy of India, Consular Wing, 2536 Massachusetts Ave, NW, Washington, DC 20008 on or after the appointment date which appears while filling up application on-line. If applying by mail, along with old passport in original please send Photocopies of all the following supporting documents should be notarized
(i) clear colored Photocopy of Green Card, Employment Authorization Document or other visa documents.
(ii) If application for Green Card/EAD/Visa is pending/being processed, document should be submitted.
(iii) Proof of Residence: Driving license or utility bill displaying applicant’s address or House Lease Agreement
